Application:  Please complete an application for each skater.  To reserve a space, applications must be signed, the consent form, the parents code of conduct form and payment must be included. Prices of the sessions are found on the brochure which is on the Schedule and Fees tab. NOTE - the link to the application has been disabled

NEW FOR THE 2011-2012 SEASON:  

Each application must include a copy of the Consent Form (1 per skater) and the Parents Code of Conduct (1 per family). Applications received without these documents will not be registered. We will be contacting those applicants that have any of the forms are missing and until Upper Canada receives said forms, the application will not be accepted and your child(ren) will not be registered to skate.

PLEASE NOTE AS OF DECEMBER 17th, 2011:

The following sessions ARE NOW FULL:

1. ALL Pre-school sessions
2. ALL Canskate sessions

If applying for a session with group lessons (CanSkate, Pre-School, Pre-Junior, Fast Traxx and Junior sessions) applications must be received 2 days prior to the first day of skating; no walk-ins will be accepted to skate the same day.

Mandatory Fees:

$33 Skate Canada Annual Membership Fee. This fee is charged by Skate Canada, not Upper Canada Skating Club. This annual membership fee expires on August 31 of each year and must be renewed every year. Skaters are not permitted to skate in any Skate Canada member club unless they have a valid active Skate Canada membership. If your application does not include $33 fee, it will not be accepted.

Fundraising fee of $25 per family is mandatory and funds collected are used to help cover the ever increasing cost of the ice, amongst other costs incurred by the Club. If your application does not include the $25 fee, it will not be accepted.

Procedure:
